Simplify DXX_ALWAYS_ERROR_FUNCTION
...
Since this should never be invoked, simplify the definition. The new
version is slightly less informative in case a logic error causes it to
be called, but is far shorter and easier to understand.

Add convenience ntstring::operator=(const char (&)[N])

OS X-specific configuration code assumes the ability to assign a literal
string to an ntstring. Add an operator= overload to support that.
